Proof of Vaccination

As a condition of participation in 2021-22 season, all volunteers (coaches, trainers, bench staff, on-ice helpers and den moms) must be fully vaccinated (2 doses plus 14 day waiting period after the second dose).

By submitting this form, volunteers can submit their second dose administration receipts to the NGHA so that their vaccination status can be verified.

Note: Carleton University has additional requirements, which are set out in the NGHA COVID-19 protocol.

Privacy Statement

The NGHA is collecting vaccine information for the 2021-22 season. It will only be collected for this purpose, and will be deleted once a volunteer’s vaccination status has been confirmed.

All information is held centrally on a secure server, and is only accessible to those NGHA officials involved in the proof-of-vaccination process.

Volunteer Screening Requirements

To ensure the safety of NGHA players, volunteers who interact with players are required to undergo a Level 3 Vulnerable Sector Screening.

Emergency Action Plans

All NGHA teams must have an Emergency Action Plan in place to deal with serious health emergencies that occur on the ice or in the arena.

The NGHA adheres to the OWHA and Hockey Canada concussion policies, and the Hockey Canada Safety Program return to play guidelines

Volunteers Needed
